    Teladoc Health Introduces Teladoc One To Deliver Connected And Adaptive Virtual Care

    Teladoc Health announced Teladoc One, a new virtual care model designed to deliver a single, predictive, and adaptive care experience built around each person

    Teladoc One is intended to address the healthcare industry’s longstanding challenge of fragmented care, where solutions are often built around individual diseases rather than a person’s full health needs. The model continuously adapts to each individual, with Teladoc Health care teams delivering and coordinating care across virtual settings and alongside a member’s trusted local providers

    The launch builds on Teladoc Health’s position as the nation’s largest multi-specialty virtual care practice. The company said it has delivered more than 100 million visits across primary care, mental health, chronic illness, and acute care

    At the center of Teladoc One is Teladoc Health’s Pulse intelligence engine. Pulse combines clinical history with available context from claims, pharmacy, device, medical record, engagement, and eligibility data to help care teams identify needs earlier, intervene at the right time, support specialty referrals, improve follow-up, and drive better outcomes

    Teladoc Health said Teladoc One is the result of two years of rebuilding its technical foundation to bring these assets together. The company is positioning the platform as a connected care model that can help employers and health plans address rising total cost of care

    The company pointed to the scale of the challenge, noting that three in four Americans manage at least one chronic disease and that chronic disease drives approximately $4.7 trillion in annual spending. Teladoc Health also noted that the average U.S. adult spends eight hours each month coordinating healthcare and regularly uses six different health-related apps

    Teladoc One is designed to treat every patient as a “population of one.” The model supports personalized care pathways that match patients to the right level of care based on cost and need, reducing the burden on patients to determine what to do and where to go next

    Under the Teladoc One model, care teams anticipate a patient’s needs and either deliver care directly through Teladoc Health clinicians or route members to local trusted providers when appropriate. When in-person care is needed, Teladoc Health care teams actively coordinate across settings to help ensure follow-through

    Patients are supported by a multidisciplinary virtual care team that spans licensed clinicians, certified health coaches, registered dietitians, mental health therapists, and specialists. They also receive support from a human care guide who helps keep them on track, coordinates with in-network primary care physicians and specialists, exchanges data, and escalates to other care team members when needed

    The model also includes always-on AI support that works alongside the human care team. The AI support is designed to keep members engaged between human touchpoints by using a member’s history and preferences to check in, send reminders, help with scheduling, and gather information for the care team

    Teladoc One is designed to improve health outcomes while lowering total cost of care through earlier intervention, better coordination, and more efficient use of clinical regh smarter medication management, optimized care site selection, avoidance of unnecessary referrals, improved condition control, and reductions in emergency room visits and hospitalizations

    As part of the model, Teladoc Health is placing 100% of its fees at risk, tying payment directly to medical cost savings and clinical outcomes for a full population. The company said this creates a more accountable partnership model with clients

    Teladoc Health said its connected care model is already showing customer value. The company noted that 67% of Teladoc Health clients have two or more products, and a study of more than 29,000 Teladoc Health members enrolled in multiple chronic care programs found that combining chronic care and mental health led to greater reductions in blood sugar and more <a href="https://healthylife7.com/county-spends-1-5m-on-glp-1-weight-loss-drugs-may-offer-employees-gym-passes-instead/" title="County spends $1.5M on GLP-1 weight loss drugs, may offer employees gym passes instead”>weight loss

    The launch is part of Teladoc Health’s broader strategy to enhance integrated care offerings and deliver more value to customers. The company recently announced partnerships with the National Basketball Players Association, Walmart, and Instacart, and expanded its flagship 24/7 Care service to address a wider spectrum of health needs

    Teladoc One was developed in partnership with select clients and will initially target populations with cardiometabolic health needs. Programs under the Teladoc One model are expected to launch with select clients in September 2026, with broader availability beginning in January 2027
