As part of a community survey conducted in July throughout Kotzebue to better understand these challenges, respondents shared consistent concerns about trust, privacy, stigma, access to care and the lasting effects of generational trauma
While this survey does not represent every person living in rural Alaska, it highlights important concerns voiced by members of our community
One of the strongest findings was that 70% of respondents felt mental health providers do not fully understand the culture, traditions and lived experiences of people in rural Alaska. For many, healing cannot be separated from cultural identity, family values and community connections. When individuals feel misunderstood, it becomes much harder to build the trust that is essential for effective mental healthcare
One hundred percent of respondents identified privacy concerns as a barrier to seeking services. In small communities where neighbors, coworkers, friends and extended family often know one another, residents often worry that others may discover they are receiving mental healthcare. Even when providers maintain strict confidentiality, the perception of limited privacy can discourage individuals from asking for help
Access to timely care was another concern. Eighty percent of respondents reported that they were unable to receive mental health services as quickly as they needed. When someone reaches the point of asking for help, delays of weeks or months can make it even more difficult to remain engaged in treatment. Timely access is especially important during periods of crisis
The survey also found that 70% of respondents believe the stigma surrounding mental healthcare remains a significant barrier in rural communities. Many expressed concerns about being judged, labeled or viewed differently if others knew they were seeking support. These fears often prevent people from reaching out until their struggles become overwhelming
Another recurring theme was the impact of generational trauma. Respondents described how historical trauma, cultural loss and experiences passed from one generation to the next continue to shape attitudes toward mental health and seeking help today. Many felt that acknowledging and understanding this history is essential to providing meaningful and effective care
These findings suggest that improving mental healthcare in rural Alaska is about more than increasing the number of providers. It also requires building trust through cultural understanding, protecting patient privacy, reducing stigma, addressing the effects of generational trauma and ensuring that care is available when people need it
This survey is not a criticism of the dedicated professionals who work every day to support mental health in our communities. Instead, it is an invitation to listen. When community members tell us they need greater cultural understanding, stronger privacy protections, faster access to care and freedom from stigma, we have an opportunity and a responsibility to respond
Improving mental healthcare in rural Alaska requires more than simply increasing the number of available services. It requires earning the trust of the people those services are meant to support. By listening to community voices, embracing cultural understanding and working together to address the barriers that still exist, we can build a system where seeking help is met with dignity, respect and hope. That is the future our communities deserve
