    Why doctors are having a tough time tackling loneliness

    Loneliness became a medical problem over the past 15 years, and a new study finds that doctors were among the last to claim it

    The medicalization of loneliness is the move to treat a social condition as something the healthcare system should diagnose and manage. It came mostly from academic researchers, government officials, and insurers, not from physicians

    That distinction decides who carries the weight of fixing one of the country’s most discussed health threats

    If loneliness is a medical condition, the job lands on clinics and doctors, who can screen and refer patients but cannot rebuild neighborhoods, shorten work hours or replace a friend who has died

    Loneliness entered the doctor’s office

    The change started with a run of alarming numbers

    Over the past two decades, researchers connected thin social ties to a higher risk of early death, and one widely cited meta-analysis found the danger rivaled that of smoking

    Social isolation, loneliness and living alone each appeared to raise the odds of dying early by roughly a third, and the harm turned up across conditions including heart disease, dementia, stroke and depression

    Isolation among older adults

    Sofia Hiltner, a doctoral student in sociology at the University of Michigan, spent years tracking how those findings changed the conversation

    Her method was plain. She interviewed professionals who work on loneliness and combed through more than a decade of news coverage, medical journal articles and academic papers

    Her interest was personal. After her grandfather died, she watched her grandmother grow increasingly isolated, with little structure and few friends in her days, which pushed Hiltner to ask why loneliness so rarely gets treated as a social problem

    When she began studying isolation among older adults, she came across clinical trials for a loneliness pill

    That discovery treated loneliness as something to medicate rather than something produced by the way people live and work

    The leap from health to healthcare

    Linking loneliness to health gave it a seriousness it had lacked. It also, Hiltner argues, created what she calls a “logical leap” – the assumption that because a problem harms health, the healthcare system is the place to solve it

    Her argument turns on that distinction. “Not everything that relates to health belongs in healthcare. Doctors can screen patients and connect them to services, but they cannot rebuild communities or shorten work hours,” said Hiltner

    The leap works because physical health carries an authority few other causes can match. Numbers do the persuading

    A problem backed by death rates and dollar figures reads as urgent and fundable, while the same trouble described as loneliness alone can sound soft or private

    Money helped cement the case. One report tied social isolation among older adults to about $6.7 billion in extra Medicare spending each year, a figure that gave hospitals and insurers a financial reason to pay attention

    Doctors were late to the conversation

    The most unexpected part of the story is who did the pushing. Physicians, Hiltner found, did not rush to claim loneliness as their turf

    The pressure to bring it inside medicine came instead from academic researchers, government health officials, and insurance companies

    The turn became official in 2023, when the U.S. Surgeon General issued an advisory calling loneliness and isolation a public health crisis and urging the health sector to act

    Declarations like that carry weight, and they helped move loneliness from a private sorrow to a public health priority

    Sociologists have long described medicalization as the process by which everyday troubles become medical conditions

    They have typically argued that it is driven by ambitious doctors, patients seeking a label, or drug companies chasing a market

    Earlier examples, from shyness to menopause to childhood restlessness, tended to follow one of those routes

    Loneliness breaks the pattern. It was never mistaken for a hidden disease, and everyone involved knew it was social

    That openness, Hiltner argues, is exactly what makes it a revealing case, because it shows how a plainly social problem can still end up defined as medical work

    Defining loneliness as medical

    Hiltner also found unease among the people doing this work and even early signs of pushback against treating loneliness as a medical concern

    Many worried about what gets lost when a is handed to the clinic

    “There is the risk that defining loneliness as medical crowds out other ways of responding to the issue. Attention, time, and re

    “On the other hand, framing it as medical may inspire more action than would have otherwise happened.”

    The stakes are practical. Time, money, and attention are finite. Every dollar spent screening patients for loneliness is a dollar not spent on the things that produce connection in the first place:

    These include affordable housing, walkable neighborhoods, community spaces, and schedules that leave room for a social life

    Healthcare can only go so far

    What the study makes clear is that loneliness did not become a medical issue on its own. People and institutions made choices to define it that way, and those choices can be examined and changed

    Hiltner hopes the work pushes policymakers to look earlier, before isolation sets in, and to weigh what the healthcare system can and cannot do

    “Healthcare can help, but it can only go so far,” said Hiltner
