Strict criteria, no price tag, and a 2027 filing leave plan sponsors watching from the sidelines

Eli Lilly will let a small number of adults with severe obesity apply for early access to its experimental weight-loss drug retatrutide before regulators approve it, though the pathway is narrow and the company has not said what the treatment will cost.
The drugmaker confirmed the expanded-access program on Monday, Reutersreported, letting patients who meet strict criteria seek retatrutide before it clears the US Food and Drug Administration (FDA).
To qualify, a patient must be at least 18, carry refractory obesity despite tolerating the highest approved dose of an obesity therapy, and live with at least two serious or life-threatening obesity-related complications for which they already receive care
The person must also be unable to join an ongoing trial, and must have discussed all standard treatment options with a doctor, Everyday Health reported, citing Lilly.
The drug has drawn attention because of its results.
Lilly said in July that an 80-week study of adults with severe obesity and established heart disease produced average weight loss of 22.6 percent at the highest weekly dose, per Reuters.
Everyday Health, citing Lilly’s phase 3 data, reported average weight loss of 25 percent over 80 weeks when early dropouts were counted, rising to 28.3 percent among patients who stayed on treatment.
For comparison, that outlet noted separate pivotal trials found average weight loss of about 15 percent with semaglutide, sold as Wegovy, over 68 weeks, and nearly 21 percent with tirzepatide, sold as Zepbound, over 72 weeks, though the studies did not test the drugs against each other.
Retatrutide is a once-weekly injection that activates three hormone receptors, GLP-1, GIP and glucagon, Lilly says, a broader target than Zepbound, which acts on GLP-1 and GIP.
The drug remains investigational and is not approved by any regulator which is running phase 3 trials across obesity, type 2 diabetes and other conditions.
Access will run through physicians rather than patients.
A treating doctor must back the request and approach Lilly, and the FDA and an institutional review board must also sign off
Lilly’s general expanded-access policy promises a response within five business days once it receives the required medical information, though doctors who spoke with STAT News described delays and little clarity on the process.
Cost and scope stay open questions.
Lilly has not said how many requests it will approve, how long treatment would continue, or who would pay for related monitoring and care, Everyday Health reported.
On pricing after approval, the company has said only that it plans to reflect the drug’s overall value to the health-care system.
Lilly plans to file for FDA approval in the first quarter of 2027, Reuters reported
As of August 4, only one retatrutide trial was recruiting in the US, for people with high-risk metabolic dysfunction-associated steatotic liver disease, Everyday Healthsaid, citing ClinicalTrials.gov.
The company has repeatedly warned that compounded or black-market versions of the drug are illegal and unsafe.
