    AI could help wipe 1 dread disease off the map, blazing a trail for hunting down all sorts of sicknesses: Researchers

    Right now U.S. healthcare has almost everything it needs to eliminate one awful affliction—and then to adapt the resulting victory for going after many others.  

    All it would take to set the precedent is responsible AI deployment, attuned government leadership, cross-sector partnerships and generational investments.  

    That, and an orchestrated targeting of the low-hanging fruit at hand—cervical cancer.  

    The cure-in-waiting for this maddeningly preventable cancer represents a “breakthrough with a failure to follow through,” physician-researchers explain in an article published Aug. 13 in JAMA.  

    Erstwhile Surgeon General Special Advisor Adam Beckman, MD, MBA, and co-author Dave Chokshi, MD, MSc, believe cervical cancer is a prime candidate for first dibs because of its 90%-plus early-stage survival rate on top of the near-perfect efficacy of vaccines for human papillomavirus (HPV), which causes many if not most cervical cancers. 

    All that’s lacking is the will to “bridge the stubborn gap between what we know and what we do in public health and healthcare,” the researchers write. 

    Generative AI, they maintain, should already be helping with “radically compressing the timeframe from breakthrough to follow-through”—not only for cervical cancer but also for any number of other diseases. 

    It won’t be easy to turn the idealistic vision into efficacious action, Beckman and Chokshi acknowledge. That’s where their call for a four-part attack comes in. The duo’s vision is to combine the might of modern AI with the human muscle of dedicated government leadership, cross-sector partnerships and capital investment

    Beckman and Chokshi comment on each of these components in some detail. Here are brief synopses.  

    1. AI makes identifying and engaging people at different orders of magnitude feasible. 

    Consider a hypothetical 43-year-old woman for whom English is a second language, the authors propose. She’s 10 years overdue for cervical cancer screening when AI flags her in a state public-health registry integrating claims data, unstructured clinical notes and free text across multiple electronic health records. An associated AI tool supporting community health workers helps arrange the appropriate doctor visit. 

    “Although each step could be done manually, AI reduces the administrative friction hindering reliable delivery,” Beckman and Chokshi write. The automated streamlining “creates the possibility of strengthening the safety net for every patient at once for cervical cancer and potentially other preventable diseases.” 

    2. Building on the momentum of HHS’s efforts to set a public agenda for healthcare AI, state governments take the baton and run with it. 

    State authorities operating Medicaid, regulating hospitals and health plans, and managing health departments are “well suited to drive public health campaigns,” Beckman and Chokshi point out. 

    “Alabama, for instance, recently launched a cervical cancer elimination campaign. A core challenge for such efforts is how to deploy cutting-edge technology while ensuring public accountability and protecting privacy and confidentiality.”

    3. A bold goal such as eliminating cervical cancer demands cross-sector mobilization

    State government may be the optimal convener, but hospitals, health plans and community-based organizations are essential for implementation, Beckman and Chokshi emphasize. 

    “Engaging patients at scale requires the clinical operations and biomedical expertise of healthcare organizations,” they note, adding that a public health lens “reveals patients who are often invisible to clinical systems and identifies creative ways to reach them.” 

    “Bridging silos across sectors would make AI tools more useful, particularly when public health [officials] and healthcare [providers] partner on data integration and delivery infrastructure.”

     

    4. Launching an agenda of breakthrough to follow-through requires up-front capital. 

    Ideally, a public agenda would be publicly funded through entities like Advanced Research Projects Agency for Health and the CMS Innovation Center, Beckman and Chokshi write. Meanwhile public capital plays a vital role in “derisking” initial investments that can be sustained by private payers in later stages of development. 

    To reach marginalized patients, they add, “government funding should particularly be directed to community health centers, behavioral health clinics, safety net hospitals and local health departments, as well as backbone organizations capable of cinching together cross-sector collaboration.”

    “The techno-utopian vision of doubling life span with AI seems a stretch,” Beckman and Chokshi conclude with a gentle snark. 

    “But should we not find the status quo—in which the average U.S. citizen celebrates only a single healthy birthday after age 65—just as unbelievable?”
