    An inflammatory bowel disease diagnosis usually comes with a long to-do list

    There are medications to start, foods to test and cut, symptoms to track, and appointments to book. Almost all of that attention goes to your gut, which makes sense, since that’s where the pain and the unpredictability show up

    But a lot of people notice something else changing around the same time. Sleep gets harder. Mood dips and hangs around longer than it used to

    In a recent Swedish study, researchers tracked mental health diagnoses in tens of thousands of people with IBD to figure out when those changes tend to start

    About the study

    Doctors have known for a long time that anxiety and depression are common in people with IBD, which is an umbrella term for Crohn’s disease, ulcerative colitis, and IBD-unclassified

    What hasn’t been clear is the timing. Most earlier research started counting after a diagnosis, so it couldn’t say much about the years leading up to one

    This study set out to fill that gap. Researchers used Sweden’s national health records to follow more than 43,000 people diagnosed with IBD between 2007 and 2023

    They compared that group with about 179,000 similar people who didn’t have IBD. They also compared patients with their own siblings who didn’t have it, which helps rule out shared genes and a shared upbringing

    Mental health diagnoses were counted from five years before diagnosis all the way to 10 years after

    The risk started climbing before diagnosis

    The shift didn’t start on diagnosis day. The chance of being diagnosed with a mental health condition began rising two to three years earlier

    It peaked about six months after diagnosis, when people with IBD were roughly 50% more likely to be diagnosed with a mental health condition than their matched peers. Researchers point to that stretch as a good window for mental health screening

    Things eased up after that, but they never went back to where they started. Ten years later, the risk was still about 19% higher

    Depression, anxiety disorders, and substance misuse made up most of that gap. Researchers didn’t see the same rise for psychotic disorders, personality disorders, or ADHD

    The sibling comparison showed a similar pattern, which suggests family genes and home environment don’t fully explain what’s going on

    It’s part of why stomach trouble can drag down your mood, and why stress can upset your stomach

    IBD involves ongoing inflammation in the gut, and that may be one piece of it. The rest is more human than biological. Symptoms often drag on for years before anyone gives them a name, which is scary and exhausting on its own

    Then comes the diagnosis, starting treatment, and living with a condition that flares on its own schedule

    Research like this can show a link between IBD and mental health conditions, but it can’t prove that one causes the other. The data also came from Sweden’s health system, so the picture may look a little different elsewhere

    What this means if you’re living with IBD

    If your mood or your sleep feels off, you don’t have to write it off as the price of a chronic illness. Bring it up with your care team the same way you’d bring up a flare. A few things worth asking for:

    • Ask your gastroenterologist whether mental health check-ins are part of your follow-up plan, especially during that first year.
    • Ask for a referral to a therapist or psychiatrist if new or worsening anxiety, low mood, or trouble coping starts touching your daily life.
    • Keep both sides of your care talking to each other, since researchers argue psychological support works best when it’s built into IBD treatment from the very first workup.

    The takeaway

    IBD asks a lot of your body, and this research suggests it can ask something of your mind too, especially in the window right around diagnosis

    A higher risk isn’t a prediction, and plenty of people never develop a mental health condition at all. But knowing the pattern is there makes it easier to name what you’re feeling and ask for help sooner
