    Weight-loss medication helps people drink less alcohol

    You’ve probably heard someone mention Ozempic in the last month, whether it was a diabetes prescription, a weight-loss story, or simply because these drugs seem to be everywhere

    Now there’s a genuinely unexpected finding involving that same family of medications: a daily pill appears to help people with Alcohol Use Disorder drink less

    In a field that’s gone nearly 20 years without a new approved treatment, that’s the kind of result people pay attention to

    Researchers from CU Anschutz focused on the oral form of semaglutide

    Their hypothesis was that taking a pill every day would be easier for someone in recovery than getting injections, which earlier studies had relied on

    Alcohol addiction needs better treatments

    Alcohol Use Disorder (AUD) is a chronic condition in which people cannot control their drinking, even as it harms their health, relationships, or finances

    It affects millions of people, yet medicine hasn’t had much new to offer them. Nearly 20 years have passed since the last medication specifically approved for AUD reached the U.S. market

    “This study suggests oral semaglutide may help reduce heavy and harmful drinking, even in people who are not trying to quit alcohol entirely,” said Joseph Schacht, an associate professor at the CU Anschutz School of Medicine

    “It could represent a new treatment option for AUD, particularly for those who have not benefited from existing medications, and may reduce alcohol-related health and social harms

    “Importantly, even reducing heavy drinking can lead to meaningful improvements for patients and families.”

    Testing a daily semaglutide pill

    Researchers recruited 50 adults, all of them already seeking help for moderate-to-severe AUD. Half got a daily semaglutide pill, half got a placebo, and nobody involved, patients or researchers, knew who had which. 

    The dose started low, 3 milligrams a day, then climbed to 7 milligrams for the second half of the study

    These weren’t people who had the occasional glass of wine with dinner. Going in, the average participant was drinking around six drinks a day and hitting roughly five heavy drinking days every week. 

    Almost everyone in the trial landed in the highest-risk categories the World Health Organization (WHO) tracks

    Semaglutide reduced alcohol use

    It wasn’t a perfect scorecard, though, and it’s worth being honest about that. 

    The trial’s most important measure, a lab test gauging how strongly people craved alcohol when shown drinking-related cues, didn’t come out statistically significant. 

    The drug also failed to meaningfully cut average drinks per day when every single day of the study got counted, sober days included. 

    Still, the overall pattern in the data pointed in a consistent direction. Participants drank less harmfully in their daily lives, even if a couple of the more controlled clinical measurements did not fully support that finding

    Participants didn’t lose weight

    Semaglutide’s whole reputation is built on weight loss. That’s not what happened in this trial. 

    “Interestingly, participants who received semaglutide did not lose weight, suggesting that the reduction in their drinking was independent from weight loss or eating behavior,” Schacht said

    This surprising finding suggests semaglutide isn’t just quietly curbing appetite and dragging alcohol consumption down along with it. 

    Something else seems to be going on, something closer to the brain’s actual craving and reward machinery. 

    A potential new treatment

    “Alcohol Use Disorder can affect nearly every part of life, including physical health, mental health, relationships and safety

    Many people do not respond to current treatments, highlighting a significant unmet medical need,” Schacht said. 

    “With no new FDA-approved medications for AUD since 2006, identifying new therapeutic options is especially critical

    For families, even modest reductions in heavy drinking can mean greater stability, fewer crises, and improved quality of life.”

    So nobody’s promising a cure here. What’s on the table is something smaller and more believable: fewer bad nights, fewer emergencies, a little more steadiness at home. 

    For a lot of families living with this, that’s not nothing. That might be everything

    Why bigger studies matter

    Side effects during the trial were mostly mild, and people stuck with the treatment at high rates, which matters a lot if this is ever going to work outside a research lab. 

    Adverse events occurred at similar rates in both groups

    However, the events researchers believed were related to the drug were more common in the semaglutide group. One person quit the study after developing a rash

    The researchers are careful not to oversell any of this. Fifty people, one research site, a study group that skewed heavily white and, by design, included only people who were already above average weight. 

    All of that narrows how far these results can really travel. The team is calling this hypothesis-generating, not proof, more of a strong lead than a finished answer

    Bigger trials, run over longer stretches of time, are the obvious next step. 

    Part of the funding came from the National Institute on Alcohol Abuse and Alcoholism. CU Anschutz and its partner institutions are already planning larger studies to see whether these early results hold up under closer scrutiny

    The study is published in the American Journal of Psychiatry
