Mounjaro is a once-weekly injectable that activates both GIP and GLP-1 receptors. Lilly markets the same active ingredient, tirzepatide, as Zepbound for obesity, making the molecule one of the company’s most important growth engines
The FDA decision covers cardiovascular death, nonfatal heart attack and nonfatal stroke in adults with type 2 diabetes who face elevated cardiovascular risk. Lilly says Mounjaro is now the first and only dual GIP/GLP-1 receptor agonist approved for that purpose
The approval was supported by SURPASS-CVOT, which compared Mounjaro directly with Lilly’s Trulicity, a GLP-1 drug already proven to provide cardiovascular benefit. Mounjaro produced an 8% lower rate of major cardiovascular events and met the study’s primary goal of non-inferiority, although superiority over Trulicity was not established. The study enrolled 13,299 patients across 30 countries, rather than the more than 30,000 cited in the initial report
The label expansion lands while tirzepatide sales are already surging. Mounjaro generated $9.94 billion in second-quarter revenue, up 91% year over year, while Zepbound sales climbed 46% to $4.93 billion. Lilly raised its 2026 revenue outlook to $85 billion to $87 billion earlier this month

Mounjaro is already expanding rapidly, so even incremental penetration among high-risk diabetes patients could add meaningful revenue. The main limitation is that Mounjaro proved non-inferior, not superior, to Trulicity on cardiovascular outcomes. Stronger uptake following the label change would reinforce tirzepatide’s blockbuster trajectory, while pricing pressure and intensifying competition from Novo Nordisk NVO remain the biggest threats to Lilly’s upside
