Facial changes after weight loss: Treatment options for hollow eyes, loose skin, volume loss

Significant weight loss can be life-changing. Whether it comes from lifestyle changes, bariatric surgery or GLP-1 medications, many patients feel healthier and more confident. But after the weight comes off, some people notice changes they did not expect
Their cheeks may look flatter. The under-eye area may appear hollow, and the eyelids may look heavier or more tired. Patients often feel better than they have in years, but their reflection does not match how they feel

Why weight loss can change the face
Facial fullness is influenced by a combination of fat, skin elasticity, bone structure and the natural aging process. When a person loses a significant amount of weight, fat is reduced throughout the body, including the face. Volume loss is often most noticeable around the eyes and cheeks, where even small changes can affect appearance
At the same time, skin that has been stretched may not always tighten fully, especially when collagen and elasticity have changed with age
“Many patients benefit from a combination of treatments,” Keenum said. “Depending on their concerns, that may include addressing droopy eyelids, excess upper eyelid skin and under-eye bags, adding fat through fat grafting to hollowed cheeks and using cosmetic laser treatments to improve skin texture and help patients look more refreshed.”
Common concerns after weight loss
Patients may notice:
- Hollowing under the eyes, often called the tear trough area.
- Volume loss in the cheeks or midface.
- Loose or excess skin around the upper or lower eyelids.
- Under-eye bags or eyelid fat that does not improve with weight loss.
This last point surprises many patients. Weight loss can reduce facial fullness, but certain fat pads around the eyelids may remain. That can create a contrast between hollow areas and puffier areas, making the eyes look tired even when someone feels well-rested
Treatment options for a more refreshed look
Because every face responds to weight loss differently, the best treatment plan depends on the patient’s anatomy, goals and skin quality
- Upper and lower blepharoplasty
For some patients, upper blepharoplasty can remove excess upper eyelid skin that creates a heavy or hooded appearance
Lower blepharoplasty can address under-eye bags, loose skin and eyelid fat that remains after weight loss. It can be paired with fat grafting to restore volume to the hollow midface and a CO₂ laser treatment to tighten the skin. Together, these treatments can help the eye area look smoother and more rested
- Fat grafting
Fat grafting may be recommended when volume loss is a concern. Using a patient’s own fat, this technique restores volume to the tear troughs, cheeks and other hollow areas of the face. Results often look softer and more natural than some temporary alternatives
- Laser treatments
Laser treatments can help improve skin texture, fine lines and mild laxity by resurfacing the skin and encouraging collagen production. In many cases, laser treatments also may be used alongside surgical procedures to create a smoother, more balanced result
Why filler is not always the best answer
For years, tear trough filler was a common option for under-eye hollowing. It can still help some patients, but it is not always the best solution after significant weight loss
In some cases, filler can create puffiness, unevenness or a heavy look under the eyes. It also may not correct loose skin or eyelid fat that needs to be addressed more structurally
“The under-eye area is delicate, and it is not a one-size-fits-all problem,” Keenum said. “For many post-weight-loss patients, surgical or regenerative approaches may provide a more natural-looking and longer-lasting result than filler.”
