    For more than a decade, GLP-1 treatment for weight loss has required a weekly injection with a pre-filled pen that needed to be stored in the fridge. This acted as a barrier to entry for many potential patients who may be needle-averse or would struggle to stick to the strict storage and administration routine

    This is beginning to change. The arrival of an oral form of semaglutide is one of the more significant developments in obesity treatment in recent years because it removes those barriers and aligns with the way many patients would prefer to take their medication

    The Obstacles Injectable GLP-1 Never Quite Solved

    Fear of needles is fairly common and it has quietly kept some patients away from GLP-1 treatment altogether. Beyond needle anxiety, injectable semaglutide also comes with practical friction. The pens require refrigeration before first use, which complicates travel and limits storage options for patients without reliable fridge space

    A Familiar Ingredient in a New Format

    Wegovy oral tabletswere approved by the Medicines and Healthcare products Regulatory Agency (MHRA) on 11 June 2026, becoming the first GLP-1 receptor agonist tablet licensed for weight loss and weight management in the UK. The tablet contains the same active ingredient as the injectable version (semaglutide) though only a small amount of an oral dose is absorbed compared with an injection. To compensate, the tablet uses an absorption enhancer that protects the medication as it passes through the stomach, and treatment follows a gradual step-up schedule, starting at 1.5mg and increasing every 30 days up to a 25mg maintenance dose, over roughly the first three months.

    What the Clinical Trials Found

    In the Phase 3 OASIS 4 trial, adults taking the 25mg maintenance dose for 64 weeks lost an average of 16.6% of their starting body weight when they adhered fully to treatment, compared with 2.7% among those taking a placebo. When the analysis accounted for participants who discontinued treatment early or added other weight loss interventions, the average fell to a still notable 13.6%, against 2.2% on placebo. Around three-quarters of participants lost at least 5% of their body weight, and close to a third lost 20% or more.

    As with the injectable formulation, gastrointestinal side effects, mainly nausea, vomiting and diarrhoea, were reported more frequently than with placebo, and these were generally described as mild to moderate

    What Pharmacists Are Hearing From Patients

    For pharmacies dispensing weight loss treatment, the appeal of an oral option is already changing the conversations patients have before starting treatment. Ana Carolina Goncalves fromPharmica, a leading UK online pharmacy, said the shift reflects something prescribers have heard from patients for years without always being able to offer an alternative. “An oral option does not remove every barrier to weight loss treatment, but it removes one of the most common ones we hear directly from patients. For people who have put off starting semaglutide purely because of the injection, having a tablet with a comparable evidence base finally gives them a route into treatment that did not exist before.”

    Not a Straightforward Substitute

    The tablet is not simply a like-for-like swap for the injection. It must be taken first thing in the morning on an empty stomach, with no more than a small amount of plain water, and patients need to wait at least 30 minutes before eating, drinking anything else or taking other oral medicines, since absorption is highly sensitive to timing. The 25mg oral dose and the 2.4mg weekly injection have not been studied head to head, so any comparison between the two remains indirect. Wegovy tablets also remain a prescription-only treatment assessed on an individual basis and are not yet available through the NHS, with any future NHS access depending on separate evaluation.

    Why This Reaches Beyond One Medication

    The significance of an oral GLP-1 treatment goes beyond convenience for any single patient. Removing the need for refrigeration and self-injection lowers the practical barrier to prescribing and monitoring treatment at scale, which matters for a drug class already reshaping how obesity is treated across primary care and private practice alike. Whether that translates into wider NHS adoption will depend on cost-effectiveness evaluation rather than the format of the medicine alone, but the direction of travel in GLP-1 development suggests injectable treatment may not remain the only serious option for very much longer.
