    LIFESTYLE modification significantly reduced intrapancreatic fat, reinforcing the role of non-pharmacological interventions in improving metabolic health and supporting strategies to prevent type 2 diabetes and pancreatic disease

    Excess fat accumulation within the pancreas has emerged as an important contributor to metabolic dysfunction and is increasingly recognised for its role in the development of endocrine and exocrine pancreatic diseases. While lifestyle interventions are already established as an effective approach for reducing liver fat, evidence for their impact on intrapancreatic fat has been less clear. A new systematic review and meta-analysis now provides the most comprehensive assessment to date, demonstrating that structured lifestyle modification can significantly reduce pancreatic fat deposition.

    Diet and Exercise Delivered Consistent Benefits

    Researchers analysed 23 interventional studies evaluating lifestyle modification programmes in which intrapancreatic fat was measured using magnetic resonance-based imaging techniques. Across the pooled analysis, lifestyle interventions resulted in a significant reduction in intrapancreatic fat and an absolute reduction in magnetic resonance imaging measured intrapancreatic fat. Statistical heterogeneity was low in both findings

    Subgroup analyses suggested that combined diet and exercise interventions produced the greatest numerical reduction in pancreatic fat, followed by exercise alone and diet alone. However, differences between intervention strategies were not statistically significant (p=0.40), indicating that a range of lifestyle approaches may be beneficial

    Implications for Diabetes Prevention

    The findings strengthen growing evidence that pancreatic fat represents a modifiable target in metabolic disease. Intrapancreatic fat has been increasingly linked to impaired pancreatic function and the pathogenesis of type 2 diabetes, making interventions that reduce fat accumulation particularly relevant for diabetes prevention and management

    Unlike pharmacological therapies, lifestyle modification offers a widely accessible strategy that may improve metabolic health while targeting multiple ectopic fat depots. The authors note that combined diet and exercise programmes demonstrated the largest numerical effect, although all lifestyle approaches evaluated contributed to reductions in intrapancreatic fat

    The review supports the incorporation of structured lifestyle programmes into strategies aimed at reducing pancreatic fat and improving long term metabolic outcomes. As understanding of the role of intrapancreatic fat in diabetes continues to evolve, these findings provide further evidence that lifestyle modification remains a key component of preventive care and may help reduce the risk of pancreatic dysfunction before disease becomes established
