1 medicines such as Ozempic may reduce the risk of serious bone fractures in people with type 2 diabetes, according to a large studythat challenges fears the blockbuster drugs could damage bone health

Researchers found that patients starting GLP-1 medicines had a 21% lower risk of fragility fractures over three years than comparable patients prescribed DPP-4 medicines — another class of diabetes medicines

People with type 2 diabetes face an elevated fracture risk from low-energy trauma — such as falling from standing height or lower — despite often having normal or higher bone density. That “may be due to lower bone strength, impaired bone microarchitecture, and accelerated bone loss,” the researchers said

After analysing health records of more than 133,000 adults aged 50-90 with type 2 diabetes in the United States between 2015 and 2022, researchers found that those taking GLP-1 medicines had the greatest reductions in vertebral fractures, which fell by 32%, and hip or femur fractures, down 30%

Importantly, the reduced fracture risk was independent of changes in body weight and blood sugar levels, suggesting the drugs may affect bone directly

The findings contrast with longstanding concerns that rapid weight loss could increase fracture risk by reducing bone density. However, the researchers cautioned that the study does not prove the drugs protect bones. Because it relied on observational health records rather than a randomised clinical trial, other factors — including physical activity, muscle strength and baseline bone health — may have influenced the results

The study also found no protective effect among people taking GLP-1 medicines without type 2 diabetes, suggesting the findings may not apply to people using the drugs solely for weight management

The authors said prospective clinical trials are now needed to determine whether the apparent reduction in fracture risk reflects a genuine biological effect and whether it persists with longer-term treatment
