    I’ve heard reports that the use of drugs like Mounjaro, Ozempic and Wegovy can cause knee pain. Is this true? And, if so, what’s going on? I thought losing weight would be good for a person’s knees

    Yes, those reports are true. But the problem isn’t actually the knee joint. And while so-called GLP-1 drugs appear to be triggering the problem, the medications themselves don’t seem to cause any sort of direct damage

    Some people who’ve lost weight using GLP-1 drugs have developed pain and numbness around the outside of the knee. These sensations can also run down the knee to the calf, and they can coincide with weakness or inability to lift the foot, a condition known as foot drop

    It turns out this isn’t a new phenomenon. Long before GLP-1 drugs, doctors saw the same condition in people who’d lost significant amounts of weight. The culprit is not the knee, calf or foot but, rather, a nerve — the peroneal nerve — that runs from the outside of the knee down the lower leg and into the foot

    The nerve wraps around a bony prominence on the outside of the knee; it lies close to the surface, sheathed only by skin and a layer of fat. With rapid weight loss, that fat can disappear, leaving the nerve vulnerable

    Activities that compress the nerve — think crossing the legs, kneeling or squatting — can cause pain and numbness around the outside of the knee and lower leg. Compression can also interfere with electrical signals that lift the front of the foot, resulting in foot drop

    This condition even has a name: “slimmer’s palsy” or “slimmer’s paralysis.” Historically, it’s been relatively rare, most often occurring after bariatric surgery or extreme dieting. But doctors are now reporting cases associated with the rapid weight loss GLP-1 drugs can produce

    As with bariatric surgery and extreme dieting, the rapid weight loss produced by GLP-1 drugs makes the nerve more vulnerable to compression

    The good news is that mild compression can be reversible, especially if recognized quickly. In one reported case, the subject recovered strength and sensation by avoiding leg crossing, engaging in physical therapy and using temporary bracing

    For anyone experiencing pain or numbness in the outside of the knee or lower leg following rapid weight loss, I’d recommend a visit to your healthcare provider, especially if the symptoms are persistent or spreading — and most urgently if you’re experiencing weakness lifting the ankle or toes

    If peroneal nerve compression is the issue, conservative approaches like physical therapy and avoiding activities that put pressure on the nerve may be enough to resolve it. In more severe cases, surgical decompression can provide relief
