Residents and workers across the Farmington Valley in Connecticut are being asked to share their experiences with mental health, substance use, access to services, and other community issues through a new health and well-being survey
The 2026 Connecticut Community Health and Well-Being Survey, conducted by Amplify, is open to adults 18 and older who live or work in communities within the organization’s service area
The Farmington Valley Health District is promoting the survey to the communities it serves
The Farmington Valley Health District serves approximately 110,000 residents in 10 member communities: Avon, Barkhamsted, Canton, Colebrook, East Granby, Farmington, Granby, Hartland, New Hartford and Simsbury

The anonymous survey takes approximately 10 to 15 minutes to complete, and participants may skip questions they do not wish to answer
- Mental health and well-being
- Alcohol, tobacco, nicotine, cannabis, and other substances
- Opioids and overdose prevention
- Prescription medications
- Childhood experiences
- Access to services and community resources
- Community connections and other factors affecting health and well-being
- Firearms
- Gambling, trading and gaming
Organizers said people do not need to have personally experienced the issues covered by the survey to participate
“Whether or not you have personally experienced these issues, we want to hear your perspective,” Amplify said in materials promoting the survey
The goal is to better understand what matters to Connecticut residents, identify community strengths and emerging concerns, inform prevention efforts and help organizations plan for future needs based on residents’ experiences
The survey is available in English, Spanish, Portuguese, French and Polish
Residents can access the survey through the QR code on promotional materials or through the survey link being distributed by participating organizations
