A familiar trafficking route is carrying a new high-value cargo, but doctors warn that hidden dangers may be more serious than the price suggests
The Friendship Bridge on the border between Brazil and Paraguay near Iguazu Falls has long been considered a route for the illegal movement of cigarettes and electronics. In 2026, weight-loss drugs are increasingly joining the list of the most profitable goods for smugglers
Between January and June 2026, Brazil’s Federal Revenue Service seized more than 76,000 injection pens, and similar substances at the border. This figure was nearly three times higher than the total volume seized throughout 2025
In the states of Paraná and Santa Catarina, weight-loss drugs became the second most valuable category of confiscated goods, surpassed only by mobile phones. Tax auditor Flávio Bernardino de Carvalho said that illegal shipments are transported by intercity buses, cars, trucks, and are sometimes brought in by tourists

Smuggling weight-loss drugs generates high profits
On the busiest days, more than 100,000 people and about 45,000 vehicles cross the Friendship Bridge. Such traffic makes inspections much more difficult. Smugglers use routes that were previously used to move weapons, drugs, ammunition, and cigarettes
The large price difference makes illegal transportation especially profitable. The tirzepatide drug Mounjaro, approved in Brazil and produced by Eli Lilly and Company, can cost more than 1,400 reais. Meanwhile, and 340 reais
Public safety expert José Ricardo Bandeira noted that injection pens and According to the tax service, a pack of cigarettes costs roughly 850 times less than a

During inspections, customs officers found the drugs beneath shoe insoles, in shampoo bottles, containers of massage gel, and specially designed vehicle compartments. Some illegal goods are transported by boat across the Paraná River
The origin of the seized drugs is often difficult to establish. Some may have been stolen from pharmacies in Paraguay. Law enforcement authorities have also uncovered clandestine production of counterfeits and supplies of goods from China
Risks of using smuggled injections
Endocrinologist Alexandre Hohl, a professor at the Federal University of Santa Catarina and director of the Brazilian Association for the Study of Obesity and Metabolic Syndrome, stressed that semaglutide and tirzepatide must be stored and transported under controlled temperatures. Peptide drugs can lose stability if temperatures rise above 30 degrees or fall below zero
When a person seeks medication outside a pharmacy, they are taking a risk. We do not know where such products come from: they are not original drugs, but copies made in unknown locations

The doctor said he has already treated patients with severe complications after using smuggled weight-loss products. One woman bought a drug in Paraguay without a prescription that was sold as tirzepatide, after which she was admitted to intensive care with lactic acidosis
Another patient began using a drug that his father had brought from Paraguay. The man experienced nausea, vomiting, and diarrhea, but did not stop the injections and increased the dosage. He subsequently required emergency medical care
Demand for GLP-1 drugs in Brazil is also growing rapidly online. Between January and June 2026, the number of online orders for such products increased by 149.3%, exceeding 1.36 million. Serasa Experian estimated the total value of these purchases at more than 2.3 billion reais
Brazil’s National Health Surveillance Agency, Anvisa, reached an agreement with Paraguay’s National Directorate of Health Surveillance, Dinavisa, to strengthen border controls. At the same time, semaglutide, tirzepatide, and retatrutide – an experimental drug not approved by any regulator worldwide and legally available only to clinical trial participants – are openly advertised in Ciudad del Este
Sales through social media and the internet are accelerating the expansion of the black market for weight-loss drugs. Medical professionals emphasize that lower prices and easy access do not guarantee either the authenticity of the medicine or the safety of its ingredients
