Why belly fat gets harder to lose in midlife
There comes a point when the weighing scale refuses to cooperate despite early morning walks, careful eating and fewer desserts. For many people, that frustrating phase begins somewhere in midlife. Clothes start fitting differently, the waistline expands, and belly fat seems determined to stay put.The common belief is that metabolism suddenly slows after turning 35. Science, however, paints a more detailed picture. Age does change the body, but it does so gradually. Hormones shift, muscle mass quietly declines, stress builds up, and genes continue influencing where fat prefers to settle. Together, these changes make abdominal fat much harder to lose than it was in younger years.The good news is that stubborn does not mean permanent. Understanding why belly fat behaves differently during midlife is the first step towards managing it more effectively.

It isn’t just about calories, your hormones are quietly changing the rulesAccording to Dr Ajay Phadke, Director, Strategic Business Development, Agilus Diagnostics, “If you’ve noticed weight around your midsection gets more stubborn after your mid-thirties, you’re not imagining it. There isn’t a sudden switch that flips precisely at 35; the change occurs gradually through midlife. A combination of hormonal, genetic, metabolic and lifestyle changes determine where the body stores energy and fat are increasingly deposited around the abdomen. This includes visceral fat, which is stored around the internal organs and is linked to risks.”One important reason is the way hormones influence fat storage.During most of adult life, women naturally store more fat around their hips and thighs because oestrogen encourages this pattern. It is considered relatively protective from a metabolic point of view. But during perimenopause and menopause, oestrogen levels begin to fall, and fat storage gradually shifts towards the abdomen.Dr Phadke explained, “Hormones play a big part, especially in women. In adult life, men tend to have more visceral fat than women, as oestrogen encourages women to store fat around the hips and thighs, a more protective pattern. This is different in perimenopause and menopause. Fat distribution shifts to the abdomen as oestrogen levels fall. Visceral fat can increase from approximately 5-8% of total body fat before menopause to approximately 15-20% after menopause.”Visceral fat is different from the soft fat just under the skin. It surrounds organs such as the liver, pancreas and intestines, and excess amounts have been linked to a greater risk of type 2 diabetes, heart disease and other metabolic conditions.

Genetics loads the gun, but lifestyle often pulls the trigger
Have you ever noticed that some families seem to carry weight mainly around the waist while others don’t? That is not just coincidence.Genes influence where the body prefers to store fat, how easily muscle is maintained, how hungry a person feels and even how the body responds to exercise. Researchers have identified hundreds of genetic variants associated with body fat distribution. While genes cannot be changed, they help explain why two people following similar routines may not achieve identical results.Still, genetics is only one piece of the puzzle. A genetic tendency does not guarantee abdominal obesity. Daily habits can either amplify or reduce that inherited risk.

Your metabolism may not be crashing, but your muscles are changing
Many people blame a “slow metabolism” for every kilogram gained after 35. The reality is more surprising.Dr Phadke said, “Most of the time, metabolism is blamed for midlife weight gain, but the belief is incomplete. Earlier theories suggest that after 30, metabolism slows by a few per cent every decade. Metabolism is stable from the twenties through the sixties, then declines later in life, once lean body mass is factored in. The bigger problem is that calorie expenditure decreases with decreased muscle mass and activity levels. Muscle is metabolically active tissue, so if you lose it, you’ll burn fewer calories at rest, and it will be easier to gain fat.”That means the body is not suddenly becoming inefficient. Instead, people often lose muscle gradually with age, especially if they spend long hours sitting or stop doing strength-based exercise.Muscle burns more energy than fat, even while the body is resting. As muscle mass decreases, the body naturally needs fewer calories. If eating habits stay the same while daily movement decreases, excess energy is more likely to be stored as fat.

Stress, insulin resistance and appetite hormones also shape your waistline
Ageing affects much more than appearance. It also changes how the body handles sugar, hunger and stress.Dr Phadke explained, “You also get abdominal fat gain due to age-related changes. Insulin resistance tends to rise with age because of less activity, increased fat storage, and ageing itself. Chronic stress can play a role as well, since cortisol has been linked to abdominal fat storage, though the effect is variable. GLP-1 (glucagon-like peptide-1), is a gut hormone involved in appetite and blood sugar regulation, and it has become a key player in weight management because of its ability to help people feel satisfied and control food intake.”Insulin resistance makes it harder for cells to use glucose efficiently, encouraging the body to store more fat over time. Chronic stress can add another layer by disrupting sleep, increasing cravings and making healthy habits more difficult to maintain.GLP-1 has attracted enormous attention in recent years because it naturally helps regulate appetite and blood sugar. Scientists are studying this hormone extensively to better understand obesity and weight management.

Belly fat responds surprisingly well when the right changes are made
One encouraging fact often gets overlooked.Dr Phadke said, “Lifestyle factors finish the picture. Less movement, extended periods of sitting and too much alcohol consumption can contribute to the buildup of visceral fat. Visceral fat is highly responsive to positive changes and is often one of the first fats we lose when we lose weight. Understanding the science behind belly fat isn’t about accepting it as inevitable, it’s about finding a way to manage it.”That means even though belly fat becomes harder to lose with age, it also responds well to consistent healthy habits.Regular strength training helps preserve muscle. Brisk walking, cycling or swimming improve calorie expenditure and heart health. Eating enough protein supports muscle maintenance, while reducing ultra-processed foods and limiting alcohol can help reduce excess calorie intake. Good sleep and stress management also influence hormones that regulate hunger and fat storage.Perhaps the biggest lesson is this: midlife weight gain should not automatically be viewed as a lack of discipline. Biology changes with age, but biology is not destiny. The body may require a different strategy than it did at 25, yet it remains capable of positive change.
