    How Virtual Second Opinions Can Help Patients and Their Physicians Move Forward

    Expertise can travel even when the patient and their care cannot

    When patients are weighing some of the biggest medical decisions of their lives, primary care physicians are often the ones helping them make sense of diagnoses and treatment options. Physicians also are charged with helping patients determine whether a major treatment plan truly fits

    Cleveland Clinic’s Virtual Second Opinions program was designed for those situations

    The program is intended for patients who already have a diagnosis or proposed treatment plan and need expert counsel on what comes next. It offers patients subspecialty review without requiring travel and offers local physicians an added layer of expert guidance without replacing their care

    “A meaningful second opinion is not simply another review of the same information,” explains Dominic King, DO, medical lead of the program. “It is the right subspecialist bringing deeper expertise to the specific decision a patient is facing.”

    Why this model is different

    Dr. King says Cleveland Clinic’s Virtual Second Opinions program stands out for several reasons:

    • Virtual access to 3,500 physicians across more than 550 advanced subspecialties
    • Deep expertise in uncommon, complex and fast-evolving conditions
    • Record collection handled by the care team, easing a major barrier for patients
    • Matching within days, often faster than waiting for an in-person specialty visit

    “Medicine has become highly specialized,” Dr. King says. “For patients with an uncommon tumor, a complex valve condition or a rare movement disorder, for example, the most valuable perspective may come from a physician who focuses on that exact condition.”

    When to consider virtual second opinions

    Virtual second opinions may be especially valuable when a patient has an established diagnosis but the next step is uncertain. Common scenarios include newly diagnosed cancer, heart disease, neurologic disorders and rare diseases

    “In cancer care, evidence can change quickly, and treatment sequence often matters as much as the treatment itself,” Dr. King says. “A disease-focused subspecialist can help patients and local oncologists decide whether new evidence, clinical trials or a different sequence should shape the plan.”

    The need is not limited to life-threatening diagnoses, he notes. Quality-of-life conditions also can benefit from deeper review. In sports medicine, for example, more precise imaging and classification can reveal that two patients with similar symptoms may need very different treatment strategies

    Supporting local care

    Cleveland Clinic’s Virtual Second Opinions program is designed to complement, not replace, care from the hometown physician, adds Dr. King. For community clinicians, it offers access to another layer of subspecialty insight when a case calls for expertise that may not be available locally

    “Within a large academic health system, physicians can easily consult colleagues with highly specialized expertise,” he says. “Virtual second opinions bring that same level of support to patients and clinicians wherever they are. The goal is not to disrupt local care, but to strengthen it with added insight.”

    How the program works

    The program is available to patients in most states, who can begin the process online

    Unlike a standard telemedicine visit, the process starts with a comprehensive record review. The care team gathers records from outside providers and institutions, then matches the patient with a Cleveland Clinic subspecialist, typically within days

    The subspecialist then provides a detailed written assessment — and, when appropriate, a follow-up virtual discussion — to clarify the diagnosis and confirm or refine treatment options. In some cases, the process can be completed within one week

    Moving forward with confidence

    Many patients come to the program with accurate but fragmented information, says Dr. King. They are seeking clarity: what their diagnosis means, which options are available and why one path may make the most sense

    A strong second opinion can provide that clarity for both patients and their physicians

    “The best second opinion does more than agree or disagree,” Dr. King concludes. “It helps patients and physicians understand the options, the evidence behind them and the clearest path forward. That clarity can make difficult decisions feel more manageable.”
