    From food noise to alcohol cravings: The expanding promise of oral GLP

    The big GLP-1 story of 2026 has been the arrival of the pills that have taken both Europe and the US by storm

    Both oforglipron (Foundayo) and the oral pill version of Wegovy are approved in the US, and already seeing millions of prescriptions, and the Wegovy pill is now available in UK pharmacies after approval in June and Europe after July approval by the European Commission

    Foundayo hit the headlines earlier this year after it was shown to help maintain weight loss after injectable GLP-1s

    But what else do we know about GLP-1 pills? Vast quantities of research has been published on the benefits of GLP-1s beyond weight loss, but the majority has been on the injectable versions

    The pills are a different matter, so here’s a snapshot of what’s known so far…

    A recent study from the University of Virginia found that GLP-1 pills can influence an area of the brain tied to reward and motivation and reach deeper into the brain than previously thought

    It is already known that GLP-1s engage regions in the brain – the hypothalamic and hindbrain regions – to suppress homeostatic feeding (eating driven by the body’s need for energy and nutrients)

    However, their effect on the neural circuits in the brain that drive hedonic feeding (eating driven by pleasure, reward or sensory appeal) remains “mostly unknown”, said the researchers

    As oral GLP-1s are going to expand access to the drugs, “gaining an understanding of these mechanisms is especially relevant,” they added

    Using gene-editing techniques, the researchers created mice whose GLP-1 receptors behaved more like human receptors, allowing them to study oral GLP-1 drugs, orforglipron (Foundayo) and an experimental oral GLP-1 called danuglipron. These are known as small-molecule GLP-1s, whereas injectable versions are large-molecule GLP-1s

    What they found was extraordinary

    When gene-edited mice were given oral GLP-1s, the already known pathways of appetite reduction were activated

    However, in parallel, activity was also seen in neurons in the central amygdala, a deeper part of the brain that’s involved with processing emotion,and dopamine-producing neurons. Dopamine is thefeelgood neurotransmitter that controls desire and reward

    “Stimulating these central amygdalar neurons curtails hedonic feeding,” said the researchers. In other words, it dampens the desire to eat purely for pleasure and this pathway was previously undiscovered

    “What we show is that these drugs can reduce not just hunger, but the desire to pursue rewarding food,” said neuroscientist Ali D. Güler, who led the study. “They’re acting on the system that makes you want the cake, not just the system that makes you feel full.”

    They also suggest that GLP-1s could be used for other substance abuse disorders

    “More broadly, our findings suggest that GLP1R agonists could have applications in disorders of dysregulated reward signalling, including substance-use disorders. As small-molecule compounds such as orforglipron – hailed as ‘a product for the masses’– move swiftly towards widespread clinical use, it will be crucial to define their long-term effects.”

    Food and beverage needs to stay abreast of developments like this to not only support users in a different way, but to be ready for the vast numbers of people that the drugs may extend to

    “This is just the beginning,” said Güler. “If we understand these pathways, we may be able to design treatments that target specific behaviors – whether that’s overeating, addiction or something else entirely.”

    Dr Nadia Ahmad, medical director of The Weight Care Clinic, agrees. “This is an interesting piece of early research because it suggests oral GLP-1 medications may reduce food noise through a different pathway in the brain from injectable GLP-1 medicines,” she says. “The research was carried out in animal models, so we can’t yet say that oral GLP-1 medications are better at reducing cravings than injections

    “However, it does provide a better understanding of how these medicines work. If future research confirms these findings, it could help explain why some patients respond differently to different GLP-1 medications. It may also support the development of newer treatments that target food cravings and reward-driven eating more effectively, rather than simply reducing appetite.”

    Another new study found that oral semaglutide may reduce drinking and alcohol cravings in people with alcohol use disorder (AUD)

    It was the first study to look at the effectiveness of pills rather than injectables, and while small, it was a double-blind, randomised, parallel-arm trial – ie gold standard

    It found that oral semaglutide significantly reduced alcohol consumption, naturalistic alcohol craving, alcohol-related problems and cannabis use. And the effects on alcohol consumption were more effective than current pharmacological treatments

    “If larger randomized trials also indicate efficacy for AUD, semaglutide and other GLP-1RAs have potential for widespread clinical uptake,” said the study authors. “Efforts to repurpose semaglutide for AUD should continue apace.”

    This is big news in a field where, in the US, only three FDA-approved, modestly effective treatments exist, and some can’t be used in those with liver damage. GLP-1s, on the other hand, do not damage the liver and may actively help it. Ozempic has been approved to treat a form of liver disease and studies of its effects on alcohol-associated liver disease are underway

    There is a fast-growing bank of evidence that GLP-1s have benefits beyond weight loss, but these two studies demonstrate that the pills are likely to change the landscape even further

    Add to that that they’re more palatable for many than injections, easier to produce, less expensive and don’t need to be refrigerated, and there’s a recipe for millions more people to start taking them

    F&B needs to be prepared for the pills helping GLP-1s penetrate society far more deeply than they have already. To survive and support that, manufacturers will need to innovate and adapt quickly to not only more users but a much wider demographic
